Planned Purchase Order Requisitions for Radioactive Materials | Center for Environmental Health and Safety | SIU

Radiological Laboratory Supervisors that have been granted permission by the Radiation Safety Officer to utilize planned purchase orders for radioactive material orders. Complete the following form for each order placed prior to its delivery to the SIU Carbondale Campus. The Planned Purchase Order has replaced the term Blanket Requisition in Purchasing.

We recommend the expected day of deliveries to be a Tuesday, Wednesday, or Thursday. Occasionally shipments due on a Monday may be shipped on a Friday, leaving the package to sit out for a weekend in delivery agency's warehouse. This presents a problem for packages that need to be refrigerated.
